Avoid vague centers such as "Project X" or "Course notes." A better center is a decision-oriented question.

2. Core Outcomes

Add 3 to 5 outcomes the handoff must protect. Examples:

  • no duplicate customer promise;
  • no broken data pipeline;
  • no repeated misconception in class;
  • no decision made without the current evidence standard.

Each important decision branch should point to a source. That source may be a document, ticket, dashboard, rubric, dataset, email, or meeting note. The concept map should not carry every detail. It should show which source supports which decision.

Example 3: Research Project Handoff

Research handoffs fail when files move but interpretation does not. A folder may contain papers, notes, transcripts, coding rules, and charts. The next researcher still needs to know which evidence is strong, which result is tentative, and which assumption shapes the analysis.

Use this structure:

  • focus question;
  • key constructs;
  • data sources;
  • coding rules;
  • disputed interpretations;
  • exclusions;
  • next analysis decision;

Use this workflow when time is short.

Minutes 0-5: Define the receiver

Minutes 5-12: Extract the core outcomes

Common Mistakes

The first mistake is mapping documents instead of decisions. A link to a document is useful only when the map explains what that document proves, changes, or controls.

The second mistake is overbuilding the first version. A 120-node handoff map becomes another file to ignore. Start with 25 to 45 nodes, then split into submaps only when the receiver asks for depth.

The third mistake is hiding uncertainty. Use labels such as "assumed," "unverified," "owner thinks," or "needs check." Trust improves when uncertainty is visible.

The fourth mistake is skipping the receiver's walkthrough. A map built by the expert alone often reflects the expert's memory, not the receiver's needs. The receiver should ask at least 5 clarification questions before the map is considered usable.
